Terms of Use

Agreement
These Terms govern FORM, the fitness and nutrition app, and this website. By downloading or using FORM you agree to them. If you do not agree, do not use the app. legal@getinform.co.
The service
FORM estimates nutrition from photos, text, barcodes and menus, tracks training and rest, and shows a daily ring. Estimates are informational. They are not lab measurements and can be wrong. Always use judgment with food, training and health decisions.
Not medical advice
FORM does not diagnose, treat, cure, prevent disease, or extend lifespan. It does not replace a physician, dietitian, or trainer. It is not a glucose, blood-pressure, or clinical device. Allergen flags are guidance. Always check the label and your own restrictions. Consult a clinician before changing diet or training if you have a condition, are pregnant, or take medication.
Accounts
People under 16 can use FORM. Keep your login safe. You are responsible for activity on your account. We may suspend accounts that abuse the service, scrape it, reverse engineer it, or harm other users.
Subscriptions
Download is free. Scanning and analysis require a subscription billed by Apple or Google. Prices are shown in the store at checkout. Subscriptions renew until you cancel in your Apple ID or Google Play account. Refunds follow Apple and Google rules, not FORM directly.
Your content
You keep rights to photos and logs you add. You grant us a license to host and process them so FORM can function. Do not upload content you do not have the right to use. Do not upload unlawful or harmful material.
Acceptable use
No automated scraping, no resale of access, no attempts to break security, no impersonation. Features that involve other people (if enabled) must be used respectfully.
Intellectual property
FORM, the ring mark, and the product design are owned by FORM. You get a personal, non-exclusive license to use the app as provided. You do not get ownership of the software.
Disclaimer
FORM is provided as is. We do not warrant uninterrupted service, perfect accuracy of scans, or fitness for a particular health outcome. To the fullest extent allowed by law we disclaim implied warranties.
Liability
To the fullest extent allowed by law, FORM is not liable for indirect, incidental, or consequential damages, or for loss of data, profits, or health outcomes. Our total liability for a claim is limited to the amount you paid us for FORM in the 12 months before the claim, or fifty euro, whichever is greater. Some places do not allow these limits. In those places, they apply only as far as the law allows. Nothing here limits liability for death or personal injury caused by our negligence, or for fraud, where that cannot be limited.
Law
These Terms are governed by the law that applies to you. You keep any mandatory consumer rights where you live. If a dispute cannot be resolved informally, the forum is the one required by the law that applies to you.
Changes and contact
We may update these Terms. Continued use after the new effective date is acceptance, except where the law requires consent. legal@getinform.co.
